Single Vision Lenses

Lenses are covered only if power is at least +/- 0.75 diopters, in either the sphere or cylinder component.

A change of lenses is a covered service only when there is a change of at least +/- 0.75 diopters, in either the sphere or cylinder component.

Bifocal Lenses

Bifocal lenses are covered only if the power of the bifocal addition is +/- 1.00 diopter or more. A change in lenses is covered if the distance power meets the minimum change requirements (+/- 0.75 diopters), or if the power of the bifocal addition is changed by at least +/- 0.50 diopters.

Change From Single Vision to Bifocal or Bifocal to Single Vision

When changing from a single vision to bifocal, the distance component must meet the minimum prescription requirement (+/- 0.75 diopters), or the resultant power of the new prescription must meet the requirement for a change in prescription (+/- 0.75 diopters).

When changing from bifocal to single vision, the new prescription must meet the requirement for a change in prescription (+/- 0.75 diopters) figured from the resultant total power of the bifocal prescription, and the new prescription must meet the minimum prescription power requirement (+/- 0.75 diopters).

The lens materials for eyeglasses available through the JAK Optical Laboratories are:

  • Plastic
    Polycarbonate. Polycarbonate lenses are avilable for all children through age 20, and adults with a prescription of +/- 2.50.
    The guidelines for what lenses may be manufactured at JAK Optical Laboratories are:
  • Plastic – no limits
    Polycarbonate – single vision, maximum is +8.50 and bifocal maximum is +8.00. Minus has no upper limit.

REMAKES & RETURNS

Production errors will be remade at our expense. However, other non-production error (i.e., dispenser errors, Doctor errors, changes in order, entry errors) will be made at the provider’s expense.

Private pay orders are subject to a 1% sales tax, unless the provider has made an Illinois Tax Exempt number available to JAK Optical Laboratories.

Frames are warranted under the manufacturer’s warranty against defects for a period of 90 days.

To get frames replaced under warranty, you must return the original part along with the invoice from the original order. Frames cannot be replaced at “no charge” without returning the defective part to JAK Optical Laboratories.

Frame samples will be provided on a consignment basis. Unreturned recalled frames
will be billed to the provider at $12 each.

TERMS AND CONDITIONS

Introduction

These terms and conditions govern the relationship between JAK Optical Laboratories and the entity described in the Account Activation Form (herein referred to as “you” or “Buyer”). By submitting the Account Activation Form and entering orders on JAK Optical Laboratories’ website the entity accepts these terms and conditions in full. These terms and conditions apply to any purchase order (whether written, telephonic or placed via a website interface) for one or more products, accessories, services or parts placed by you. Any order submitted by you is subject to acceptance by JAK Optical Laboratories.

Prices and Payments

Prices stated are net of any taxes applicable to the goods sold F.O.B. JAK Optical Laboratories’ plant. JAK Optical Laboratories’ invoices or monthly billing statements will add any applicable taxes imposed by local and federal government and cost of shipping and insurance to the goods. Invoices shall be submitted to BUYER by JAK Optical Laboratories upon shipment of products or at the end of each month as agreed upon by both parties. BUYER shall be responsible for all taxes, handling and shipping charges, including insurance, whether billed at the time of shipment or not. Title to and risk of loss of products shall pass to BUYER upon delivery to a common carrier or private carrier for shipment to BUYER.

BUYER agrees to pay JAK Optical Laboratories at the designated address referred to on the monthly billing statement. Payment is due, and must be received, net thirty (30) days from the date of monthly statement. Any dispute concerning the amount of any invoice must be raised within thirty (30) days of the receipt of the invoice and BUYER must use its best efforts to resolve any such dispute within sixty (60) days of receipt of the invoice. All amounts due and owing not received within the stated time period will be considered delinquent and will be subject to a late payment penalty at the rate of 1.50% per month or the highest rate permitted by applicable law, whichever is less, from the due date until paid in full.

Should it be necessary to refer to this account for collection, BUYER agrees to pay reasonable attorney, court and collection fees, including on appeal. Furthermore, both parties agree that should legal action be initiated or received, such action will be governed by the laws of the State of Illinois (excluding the laws thereof with respect to conflict of laws), and any such action will be pursued in the jurisdiction of the appropriate court in Peoria County, Peoria, Illinois.

Damages/Defects

Should BUYER receive product from JAK Optical Laboratories which BUYER claims is damaged or defective, BUYER must make JAK Optical Laboratories aware of the claim within thirty (30) days. Otherwise, all claims are waived. Incorrect seg heights and scratches to lenses fall under this policy.

Prescription Changes/Dispenser Errors

Should BUYER need to submit an order due to a prescription change or dispenser error, BUYER shall be charged 50% of the normal charges for said order.

Cancellation

Once BUYER submits an order on JAK Optical Laboratories’ website, BUYER acknowledges that the order may not be subject to cancellation if the order has begun processing. If processing has begun the order cannot be cancelled.

Warranty

Frames are warranted under the manufacturer’s warranty against defects for a period of 90 days.

License to use Website

Unless otherwise stated, JAK Optical Laboratories and/or its licensors own the intellectual property rights in the website and material on the website. Subject to the license below, all these intellectual property rights are reserved.

You may view, download for caching purposes only, and print pages from the website for your own personal use, subject to the restrictions set out below and elsewhere in these terms and conditions.

You must not:

republish material from this website (including republication on another website);
sell, rent or sub-license material from the website;
show any material from the website in public;

Without prejudice to JAK Optical Laboratories’ other rights under these terms and conditions, if you breach these terms and conditions in any way, JAK Optical Laboratories may take such action as it deems appropriate to deal with the breach, including suspending your access to the website, prohibiting you from accessing the website, blocking computers using your IP address from accessing the website, contacting your internet service provider to request that they block your access to the website and/or bringing court proceedings against you.

Variation

JAK Optical Laboratories may revise these terms and conditions from time-to-time. Revised terms and conditions will apply to the use of this website from the date of the publication of the revised terms and conditions on this website. Please check this page regularly to ensure you are familiar with the current version.

Severability

If a provision of these terms and conditions is determined by any court or other competent authority to be unlawful and/or unenforceable, the other provisions will continue in effect. If any unlawful and/or unenforceable provision would be lawful or enforceable if part of it were deleted, that part will be deemed to be deleted, and the rest of the provision will continue in effect.

Entire agreement

These terms and conditions will be governed by and construed in accordance with the laws of the State of Illinois, and any disputes relating to these terms and conditions will be subject to the exclusive jurisdiction of the courts of Peoria County, Illinois.
